Intake Specialist, PRN (as needed)

Location: Alive Hospice campus, Nashville, TN

Status: PRN

SUMMARY

Under the direction of the Director of Intake & Admissions, the Intake Specialist responds to inquiry and intake calls, assists with administrative duties and is cross-trained for other provider relations roles. The Intake Specialist supports the admission of patients, communication with providers (internal and external), data entry and documentation.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Represents the agency as assigned to promote services and co-markets with provider relations representatives.
  • Serves as back-up to Director of Intake & Admissions as needed.
  • Serves as liaison and provides feedback from referral sources.
  • Collaborates with physicians, social workers, discharge planners, and clinical coordinators to gather all available information for treatment plans (discharge orders, etc.).
  • Answers and responds to multiple calls including inquiry, intake, family and healthcare providers.
  • Gives accurate, consistent, compassionate attention to each call.
  • Maintains patient, family and staff confidentiality.
  • Files, faxes, logs and maintains data in an accurate and readily accessible fashion.
  • Enters data, manipulates and retrieves data in proper reporting structure.
  • Routes paperwork appropriately and in a timely manner.
  • Assists with administrative duties as assigned by Director of Intake & Admissions Director of Clinical Partnerships
  • Other duties may also be assigned.
    SUPERVISORY RESPONSIBILITIES

    No supervision of agency staff or volunteers is required.

    LANGUAGE SKILLS

    Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization. Ability to respond to common inquiries from customers, clients or other employees of the organization. Must have computer and telephone skills.

    MATHEMATICAL SKILLS

    Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

    REASONING ABILITY

    Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

    CERTIFICATES, LICENSES, REGISTRATIONS

    If required to drive to carry out the duties of this position: current driver's license and automobile insurance as required by Tennessee state law.

    Requirements

    CONTINUING EDUCATION

    The agency requires this position to complete 6 hour of continuing education per year covering topics that will contribute to improvements in carrying out the above responsibilities. Regulatory agencies may require some disciplines to have additional hours in order to be licensed or certified.

    EDUCATION and/or EXPERIENCE

    Associates Degree (A.A.) from college or technical school; one year's related experience and/or training; or equivalent combination of education and experience.
