- Job Requirements
- BLS Certification required
- Nursys licensure verification (specific to Michigan hospitals)
- Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals
- Current license activity must be reviewed in all states
- MMR Vaccine or Positive IGG Titer required
- Fit Test required annually and can be completed onsite
- Hepatitis B documentation of vaccination series completion or declination
- Influenza Vaccine annually
- Physical license copies are not required
- 2 Step PPD OR IGRA (Quantiferon/Tspot) within 1 year or annual negative titer with lab printout
- Varicella immunity evidence or titer required
- Tdap vaccination required
- Ascension Criminal Background Check Results
- Primary residency verification for RNs/LPNs/PTs working on a compact license
- Ascension Medical Surgical Exam required
- Ascension Skills Checklist completion required annually in specialty
- Ascension 12 Panel Drug Screen within 30 days prior to start and annually
- Ascension Education Verification for accuracy and gaps
- Ascension EKG Exam with specific scoring criteria
- Ascension Core Competencies courses
  - Unit Specific Information
  - Ascension Unit Specific Checklist optional
  - Ascension Profile must be submitted despite being optional
  - Ascension 2 Professional References required prior to the first assignment
