Developmental Disabilities Services REPORTS TO: Director of
Developmental Disabilities Services INTRODUCTION: Volunteers of

support for individuals with developmental disabilities, healing

QUALIFICATIONS: This position will be responsible for the overall
services for those individuals receiving Residential Habilitation
or Supported Living services from Volunteers of America Mid States.
- The implementation of the Individual Support Plans and the
attainment of the individual's visions within their home and
community environments is essential. - Position will be responsible
for case management and referral to resources in the community as
well as direct supervision of house manages in each residential and
supported living setting. WHAT YOU SHOULD HAVE FOR THIS ROLE: BA
degree OR an equivalent number of years of progressively more
responsible direct experience with the consumer population AND a
current state driver's license. Proactively looks for potential
issues and resolves them before they can impact the organization.

external contacts. - - RESPONSIBILITIES: A. Ensure compliance with
DMH/DD Quality Enhancement guidelines for residential supports and
services. B. Ensure cleanliness and safety of clients served and
their homes. C. Ensure implementation of Individual Support Plans
and that monthly reviews encompass pertinent information that is
exchanged with the Medical and Day Service Coordinator as well as
pertinent persons from the client's families, guardians,
Independent Support Coordinators, Regional Office personnel, etc.
D. Complete comprehensive monthly reviews in a timely manner. E.
Monitor outcomes to ensure that the agency is actively striving
towards reaching the client's vision. F. Monitor the client's
personal records to ensure information contained is in accordance
with DMH/DD guidelines. G. Coordinate all residential employee
training and assist in tracking training to ensure best practices
and DMH/DD compliance. H. Coordinate with clients served,
guardians, families, Independent Support Coordinators day services,
etc. I. Participate in management meetings and supervisory planning
sessions. J. Ensure that clients served receive all possible
benefits for the purpose of increasing their disposable income
(food stamps, MR housing, level billing, discount utility programs,
STRAP, etc.). K. Implement all agency supervision and quality
assurance plans as they are pertain to residential serves and
supports. L. Ensure that prudent financial residential spending
occurs and all appropriate documentation regarding expenditures
meets policy and procedure requirements. M. Performance quality
improvements (POI) duties as assigned by supervisor and POI
