How you’ll make an impact in this role - Assess patients’ physical and emotional needs and deliver timely interventions that support recovery and positive health outcomes. - Develop and manage individualized care plans, closely tracking patient progress and adjusting care as conditions change. - Coordinate care across physicians and the broader care team to ensure patients receive seamless, well-organized treatment. - Communicate patient updates and carry out treatment plans to keep care accurate, timely, and aligned with provider guidance. - Prepare patients and families for discharge, providing education and coordinating next steps to support a safe transition home or to the next level of care. What minimum requirements you’ll need Licensure / Certification / Registration: - Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required. Education: - Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
