The Home Health Case Manager Position We are looking for a full time RN for the position of Home Health Case Manager for our Columbia office . This person will be responsible to manage and coordinate appropriate home care services from admission to discharge. This position plans and coordinates the delivery of each assigned patient's plan of care in accordance with physician orders.
