Information $2,141 per week About The Position Job Title:
Registered Nurse Profession: Nursing Specialty: Hospice Care
Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8 hours
Experience: Minimum of one (1) year experience as a Registered
Nurse in acute care hospital, oncology, long-term care, hospice, or
home health. License: Current license to practice nursing in the
state of employment. Certifications: BLS (American Heart
Association), current automobile insurance, and valid driver's
license. Must-Have: Knowledge of terminally ill patients and their
families along with understanding of hospice care; ability to apply
knowledge of the special needs of hospice patients and families.
Description: The Registered Nurse (RN) will provide direct patient
care services to hospice patients and supportive care to the
patient's family. The RN will manage an interdisciplinary group
consisting of Licensed Practical/Vocational Nurses, Certified
Nursing Assistants, and support staff to enhance the quality of
hospice care. The RN will case manage all assigned patient clinical
records and assure the highest quality of care is provided and
documented in accordance with the Plan of Care. The RN will perform
initial and ongoing assessments of patients. The RN will help
develop and implement the Hospice Plan of Care as prescribed by the
physician in conjunction with other members of the
Interdisciplinary Group. The RN will work closely with other team
members to provide physical, psychosocial, educational, emotional,
and spiritual services to achieve the highest quality of care. The
RN will report any changes in the patient's condition to both the
Attending Physician and Hospice Physician. The RN will maintain
accurate and updated medical records, including all notes, Plans of
Care, assessments, and any other relevant information. The RN will
practice infection control measures and standard precautions. The
RN will monitor the scheduling of all staff and participate in
Quality Assessment and Performance Improvement Programs. The RN may
be requested to participate in the agency on-call schedule. The RN
will consistently promote the organization's core values and
complete required Compliance annual training. The RN will perform
other assigned activities and attend staff meetings. The RN will
maintain a high level of knowledge pertaining to managing pain and
symptoms in the dying process. The RN will make routine supervisory
visits and follow up on assigned patient clinical records to assure
quality services are provided. The RN will maintain close contact
with the patient's family to provide information, support, and
continuity of care. The RN will act as the primary liaison between
the physician, patient/family, and hospice team to ensure awareness
of all clinical issues, staffing, and scheduling concerns. The RN
will be responsible for educating the patient, family, or
caregivers regarding needs identified on the Plan of Care. The RN
will prepare written instructions for patient care and make
supervisory visits to assess the competence of the Aide in
providing care. The RN will determine whether care goals are being
