REPORTS TO: QA Director/ Contracted RN
LOCATION: Nashville, TN
OVERVIEW:
The LPN Case Manager is responsible for development of and the
implementation of the health care service plans for individuals
with developmental disabilities served through the Tennessee
Community Services Waiver. - In addition, the LPN Case Manager is
responsible for the administration of such direct nursing services,
coordination with other medical professionals, and any and all
medical oversight as required to fully meet the medical needs of
the service recipients in accordance with Department of
Intellectual and Developmental Disabilities guidelines.
REQUIREMENTS:
The position requires current license as a Licensed Practical Nurse
in the State of Tennessee and at least two years of nursing
experience preferably working with persons with intellectual and
developmental disabilities or in a home health environment. Must
have experience supervising LPN's working in the homes. - Must have
knowledge of the rules, policies and procedures, and guidelines
established by the State of Tennessee for community support
programs for individuals with intellectual and developmental
disabilities, and be able to complete required DIDD training. -
RESPONSIBILITIES:
--- Develop and Update Health Passports and Health Care Plans for
each person served on the schedule required by DIDD guidelines.
Educate support staff on Health Care Plans for each person
supported.
--- Monitor, assess, and document the continuing health condition
of the persons on your case load through home visits. Document
results of home visitation in the Nurse Workload Section of the
Practical Health System. -
--- Monitor and review PHS for monthly weights, medical charting, -
documentations, E-Mar and vital signs and report any discrepancy
and report to the RSC
--- Update and send nursing orders to the ISC 90 day prior to the
New ISP date. -
--- Track Psych appointments and ensure that all paper work is
received after the apt. - Obtain verbal consent document date and
time of verbal consent and then send consents to the conservator
for signature. This is to be completed for any increase or new
psych medications..
--- Continuously assess the effectiveness of nursing procedures and
implement any changes necessary to insure quality nursing care.
--- Serves effectively as a consultation resource for staff and
clearly communicate your clinical expertise.
--- Complete, or assure the completion of, the PSR risk tool as
required by DIDD.
--- When necessary, effectively train any additional home, nursing,
and health service personnel required to meet the needs of the
persons supported.
--- Effectively works to maintain DIDD standards and takes prompt,
effective action to correct any deficiencies. -
o Attend required Circle of Support meetings on a scheduled basis
and effectively use these to
o resolve problems, review nursing issues, and to make
recommendations for changes in the -
o persons supported Individual Support Plan. -
--- Follow the Monthly Medication Protocol to ensure that all
required medications are available on a timely basis for each
person served. Monitor the individual's use of medication according
to need, including the monitoring of side effects, educating staff,
and evaluating treatment results.
--- Make recommendations for improvements for non-nursing services
which impact the administration of nursing and other health related
services. - May coordinate and implement the education of other
non-nursing staff on specific health related issues such as safety,
infection control, and preventative practices. - -
--- Ensure completion of medication variances are being
reviewed.
--- Be available and on call 24/7 for consultation and/or treatment
of minor illnesses or injuries as needed.
--- Participate in the initial evaluation for all new admissions
who well be served in the program.
--- Complete the health passport, health care plan, and assessments
for all individuals in the residential supported living program.
Establish medical records for each individual served in the
residential supported living program. -
--- Perform health care procedures which are ordered by a physician
or other medical practitioner as required by standards of
professionalism practice or law to be performed by licensed nursing
personnel.
--- Ensure that quality care is given as required by funding
regulations and that the agency complies with survey criteria as
they relate to health services.
--- Supervises the LPN Health Services Coordinator and LPN's in the
homes.
--- Completes LPN's schedules in the Medical Supported Living homes
and posts the schedules in each house. - -
--- Works in the home to help reduce OT. -
--- Conduct agency TB testing/ TB Assessment for all staff and
maintain updated records.
--- Prepare Annual Nursing Plan of Care and Nursing order for all
persons supported medical supported living and send to ISC no later
than 90 prior to the ISP Date. -
--- Track and Prepare Annual Medication Orders and Standing orders
for all persons supported.
--- Any other responsibilities as assigned to meet the mission of
the agency.
--- Performance quality improvement (PQI) duties as assigned by
supervision & PQI committee.
