Job Summary: * You will address the discharge planning needs and the psychosocial needs of the patient population served by the facility, and those patients referred through an open referral policy as well as those referred through high risk referral criteria. * You will render assessment, community service coordination and assist with discharge planning to facilitate continuity of care and the transition of patient with personal/environmental difficulties that may interfere with obtaining maximum benefit from medical carServing our community for more than 30 years,--HCA TriStar Southern Hills has been a leader for a wide range of healthcare services, including emergency services, cardiology, orthopedics and rehabilitation.
